TRT and Hormone Titration

Review of pre-visit lab results

Continuation of care (with refill of medications) requires appointments every 3 months

Pre-visit labs are required prior to initial, 3rd, 6th, 9th, and 12th month appointments

$100 deposit prior to visit will be applied to $150 total visit fee if doctor and patient decide to move forward with this service

$150 for all follow up visits

Erectile Dysfunction
Visit Includes:

Diagnosis and discussion of safe care options, such as:

Tadalafil (Cialis)
-
Sildenafil (Viagra)
-
Alvanafil (Stendra)
-
Phentolamine, Papaverine and Alprostadil (TriMix)

Review of patient history to avoid medication interactions

Determination of treatment plan and teaching

Appointments required every 3 months for evaluation of side effects and treatment efficacy

$100 for the initial and $50 for all follow up visits
Sexual Health Visit
Visit Includes:
-
Diagnosis & treatment for sexually transmitted infections (STI)
-
Evaluation for Doxycycline post-exposure prophylaxis (doxy-PEP) treatment
-
Evaluation for HIV pre-exposure prophylaxis (PrEP) treatment
-
$100 for all sexual health visits
-
PrEP continuation of care (with refill of medications) require appointments every 3 months for the first year followed by 6-month intervals thereafter. Pre-visit labs are required prior to each visit. PrEP is encouraged to be taken regularly to maximize effectiveness.
Primary Care
Visit Includes:

Personalized establishment of care with determination of health goals

Review of medical history and current medications

Physical examination
-
Management of chronic and new conditions
-
Full coordination of care (lab studies, imaging, referrals, counseling & therapy)

Inclusion of all services offered by Myo Men’s Health (TRT, ED or Sexual Health)

$200 initial (enrollment) fee, $150 continuation of care (with refills of medications) fee every 3 months thereafter, $100 post-emergency department or hospitalization follow up fee, $100 acute (new) condition visit

​Visits to discuss studies ordered by Myo Men’s Health will not incur additional costs

Physical Therapy Referral
Visit Includes:

Review of symptoms and medical exam
-
Assessment for contributory medical conditions or physical findings warranting additional workup ​

Determination of need for PT and completion of required referral paperwork to your therapist of choice
​

$50 for all physical therapy referral visits
